On Friday, Aug. 2, the Center for Elders’ Independence (CEI) celebrated its expansion in West County with the grand opening of its new PACE Center (Program of All-Inclusive Care for the Elderly Center) at 3645 San Pablo Dam Road in El Sobrante.
The facility began enrolling participants in May and is now fully operational, offering comprehensive healthcare services to seniors in the region. It serves older adults in Richmond, San Pablo, El Cerrito, El Sobrante, Pinole, and surrounding neighborhoods.
CEI’s PACE model provides all-inclusive medical, behavioral, and social services through one coordinated care plan, enabling seniors to remain independent and avoid unnecessary nursing home placement.

“We are proud to bring CEI’s trusted model of care to West County,” said Maria Zamora, CEO of Center for Elders’ Independence. “Too many elders here have been left behind—facing language barriers, long commutes for care, and the devastating effects of isolation. Our El Sobrante center is a place where older adults can get the healthcare and support they need to thrive in their own homes and communities.”
Services offered at the CEI Tri-Valley PACE Center include primary and specialty medical care; physical, occupational, and speech therapy; in-home care and personal support services; behavioral health and counseling; transportation to and from the center and medical appointments; nutritious meals and dietary counseling; medication management and pharmacy services; vision, dental, and podiatry service; and social and recreational programs to build connection and joy.
CEI now operates six PACE Centers in San Leandro, East Oakland, Oakland, El Sobrante, Livermore, and Concord.
